Scout Program Registration Form
Payment Information:
Fee of $5 per Scout includes an interactive tour of the museum and a Carlyle House participation patch. The first
two (2) adult chaperones are free of charge. Additional adults are $5 each. There is a $25 minimum payment
per program. Full payment is due two (2) weeks prior to the program.
If you wish to cancel or reschedule a program, Please Call 703-549-2997 at least one (1) week prior
to your scheduled program. No refund will be given for cancellations made less than one (1) week before
the scheduled program date.
